Is your staff fully licensed and trained in memory care? View Answer Close Answer

Yes. Each of our family-owned communities provides the peace-of-mind that your loved one is surrounded by fully qualified staff and will receive the following services:

  • Staff trained specifically for Alzheimer’s and dementia-related memory care on-site 24 hours a day
  • 24-hour security with monitored and magnetically locked exterior doors
  • Emergency call response in each room
  • Administration and storage of all medications – with options for re-ordering
  • Health monitoring (vital signs, lab tests, therapy needs, glucose levels)
  • Assistance with daily living including bathing, dressing, eating, toileting, hygiene and mobility
  • Laundry – personal and linens
  • A full-time, dedicated housekeeper who cleans the entire community including each resident’s bedroom and bathroom

If my loved one lives out of town, how can I relocate them to the Autumn Leaves community? View Answer Close Answer

We are here to assist you should your loved one need to relocate to an Autumn Leaves community. Our highly trained assessment team will travel to your loves one’s current residence to meet with you and your loved one to make the transition as easy as possible. We understand this can be a stressful time for families, and our assessment team will travel with your loved one to ensure their safety.

Are visits with my loved one welcome at any time? View Answer Close Answer

Yes. As you would visit your loved one in his or her own home, you are welcome to visit your loved one at any time in their new home with us. If your loved one is accustomed to you visiting at a certain time of day, we encourage you to continue that routine as consistency benefits your loved one.

As part of your Alzheimer's care do you monitor my loved one’s nutritional needs? View Answer Close Answer

Our dietary staff works closely with the Director of Health Care and Community Director to ensure every meal provides the highest levels of nourishment for our residents.

  • Our staff is committed to providing the closest to home cooking as possible through freshly prepared meals made from scratch
  • Three nutritious meals prepared daily with attention to individual dietary needs
  • Snacks are available 24 hours a day
  • Monitoring of weight loss and weight gain
  • Ongoing evaluation of nutritional needs and special dietary requirements
  • Monitoring of glucose levels for diabetics (when prescribed by a doctor)

Can you describe the rooms at Autumn Leaves? View Answer Close Answer

We offer both private and semi-private rooms with bathrooms in all rooms. Pull cords are available in all bedrooms and bathrooms. All of our rooms are carpeted and spacious, with a variety of sizes of rooms available. You are welcome to bring your own furniture if you like or we can provide furniture for you.

Each of our buildings have been purposefully built to reflect an inviting home environment. These unique community designs have been thoroughly researched to provide the best accommodations for individuals living with Alzheimer’s disease and other forms of memory impairment.

  • Themed neighborhoods with unique colors and images, extra-high ceilings, and wide hallways
  • Numerous activity areas and rooms for TV viewing, arts & crafts, reading, visiting, and private dining with the family
  • Specialized design features that minimize confusion, encourage social interaction, enhance visual stimulation, and provide a home-like atmosphere
  • Enable our residents to feel secure through appropriately placed handrails, security and monitoring systems, and themed neighborhoods

Our certified Life Engagement Manager understands that in addition to medical care, incorporating a holistic approach to our life engagement program is critical to the wellbeing of our residents. Life engagement does not stop at 5:00. Our program is designed to engage our residents seven days a week.

  • Our Life Engagement program uses touch, laughter, music, and dance on a daily basis to enrich the lives of our residents and keep them active
  • Family incorporated activities, cookouts, birthday parties, and holiday-themed dinners and events
  • Gardening, field trips, and supervised outdoor walks in our courtyards and walking paths
  • Personalized one-on-one programs that help residents continue to enjoy their life-long hobbies such as cooking, needlework, gardening, and painting
